The COS building is located across from North Shore University Hospital. Please plan to park for up to two hours for a
new patient visit.

From the North:
Take exit 34A for US-41 S toward Skokie Road. Take Skokie Rd for 1.1 mile, turn left at Glenview Rd for 0.8 mile,
turn right at N Crawford Ave for 0.6 mi, turn left at Central St for 2.3 mi, then turn right at Ridge Ave. Building
is on the right.

From the South:
Take exit 35 for Old Orchard Rd. Turn right at Old Orchard Rd for 1.2 mi, turn left at Gross Point Rd for 0.2 mi,
turn right at Central St for 2.3 mi, then turn right at Ridge Ave. Building is on the right.
From Lakeshore Drive:
Take Lakeshore Drive to Sheridan Road, then turn right on Devon. Follow it for 0.8 mile,
and turn right on Ridge for 4.6 miles. Building is on the left.
CTA Bus:
Take the Central/Ridge Bus to the intersection of Central and Ridge. Turn left on Ridge Ave.
Building is on the right.
CTA Rail:
Take the Purple Line to the Central Stop. Walk one block east on Central, and turn right on Ridge.
Building is one block south on Ridge, on the right.
Parking
Lot parking is available for $2 for the first hour and $1/hour thereafter. The entrance is on
Ridge Ave just south of Central St.
Street parking is residential permit only.

The Illinois Masonic Medical Office Building is located in Lakeview. Please plan to park for up to two hours for a new patient visit.
We are sharing space with Chicagoland Complete Healthcare, Dr. Tohfa Ruda and Dr. Anita Bednarz.

From the North:
From 94-E, take exit 46A for California Ave and turn left onto Diversey Ave. Continue for 2.5 miles and turn left onto N Halsted. The building will be on your left.

From the South:
From 94-W, take exit 48B to North Ave. Turn right on North ave and continue for 0.8 miles. Turn left onto N. Halsted Street, and continue for 1.8 miles The building will be on your left.
From Lakeshore Drive:
Take the Belmont Ave exit for 0.5 miles. Turn left onto N. Halsted St. The building is on the right, as is a parking lot.
CTA Bus:
Take the 8 bus and exit at Halsted and Wellington. Or you may take the 22 or 36 bus to Clark and Wellington. Walk one block west to Halsted. Building is on the northwest corner of Halsted and Wellington.
CTA Rail:
Take the Brown or Purple Line to Wellington. Walk two blocks east on Wellington to Halsted. Building is on the northwest corner of Halsted and Wellington.
Take the Brown, Purple, or Red Line to Belmont. Walk one block east to Clark, then one block southeast to Halsted and Wellington. Building is on the northwest corner of Halsted and Wellington.
Parking
Lot parking is available for $4 with validation.
Street parking is Pay Box only.
